How does retrieving items from your grave work?
I'll spare the story for those that actually want to hear why I'm asking, but if you die before retrieving your items, do they disappear or do you get another gravestone?
The reason why I'm somewhat confused is I had not died twice in a row before retrieving my old tombstone before until today. What happened was my tombstone was filled to the brim with items in every slot, and in my absolute infinite wisdom I didn't realize how absolutely moronic it'd be to pick up some items on my way to my tombstone. Due to this not only was I stuck on the menu, but a few items were left in the tombstone. While doing that, I died again. I kind of assumed I lost the items that were left at my old tombstone, but when I went back I had two tombstones, one with the leftover items and the items I had picked back up, and things were fine. I honestly can't tell if this is just normal, or if there's some reason why it happened like this, like maybe I got a second tombstone because I had died very quickly after that first death, or something like that.

Well I have never experienced what you say in that once I loot my tombstone ( I'm buried there ) it disappears and is replaced with a skull and crossbones which stays on the map forever. Tombstone stays on map forever, tombstones will only generate though if you have something in your inventory. There's a few more details to know but I think this answers your main query for now

Note no skull on mini map yet

once I get my gear back

This is good to know. I've been meaning to ask how long a tombstone hangs around. About 600-700 game days ago I had an unfortunate incident involving a mountain, a blizzard, a cliff and free-fall (splat!). Then I did something stupid, I didn't put down and extra mark where the tombstone was. Died four more times, again, no mark. I know the general area they are in, just a matter of going over it with a fine tooth comb. Lost my upgraded Wolf/Iron/bronze and Troll armor and upgraded weapons (eventually found the Troll armor). The cliff emptied out on a large plains. I had to re-kill Bonemass to get another wishbone.
I was angry and depressed with myself for being so stupid I bought a one pound bag of potato chips and a large coke and ate it in one afternoon...Anyway, I kept playing, got the padded armor and killed off the other bosses. This gives me something to do until an update comes along. I was getting tired of wandering the biomes at night stocking up on food and ore and killing all the snotlings I could find.

For a given value of 'permanently' yes - there are places to die that make retrieval something of a long term project. Try not to leave the map entirely when dying.
Yes, for example in the mountains during a "you are being hunted" event. 😢

Plains at night, that's a good way to die.

You go to your gravestone and hit the letter "E" if on pc. At that point you get all your stuff. If you die on the way, you make a new stone but your old stone still has your stuff and you need to find it.
Luckily there is a time period where you can't lose your skills from dying a 2nd, 3rd, or in the case of my friend 30+
Anything else is a bug and should be treated as such.Alright so what I'm getting from this is I don't risk losing my items permanently by death? That's actually great news, I like being punished for my dumb mistakes but losing my items I've worked hard on feels a bit heavy. Also thank god we have that skill loss grace period, the amount of skill levels I've lost after discovering the Swamp is already in the double digits. Tombstone stays on map forever, tombstones will only generate though if you have something in your inventory. There's a few more details to know but I think this answers your main query for now
So far, the only real way to lose everything permanently is to go off the edge of the world some how, but even then, if you are quick enough... log out, drop gear off in another world, log back in, die, and then go get your gear when you respawn.

But keep in mind, that your previous tombstone marker will disappear. If you suspect you might die twice, I suggest you to place another marker by hand, so you can die as much as you want without losing the position of what you're actually trying to rescue.

You get another gravestone each time you die,
If you do not totally empty your gravestone it stays until you do. I've accidently come across some of my gravestones where I was killed while running to my initial death point. Had forgot about them after I got yo first death stone.
ALWAYS mark your initial death on map as system drops their mark if you die in corpse run.

Pro tip: if you've died in a particularly dangerous area with your gear, mark where it is on your map. That way if you die again you don't lose the location of your good stuff
