I don't think I have ever had any mob spawn on top of me as I wandered about.

However, if I was killed by mobs they often remain in the area. So on corpse runs I always expect whatever killed me, minus some environmental death, to still be there and prepare appropriately.

Sounds like you might have tried a naked corpse run?

There's nothing to fix because nothing is broken.
