Its not holes in the coverage that is certain. Nothing else ever spawns inside just deer and birds and those shots aren't from the same base or world seed though all of them are meadows bases.

Deathsquitos and fulings can spawn outside your work bench area and travel inside it. Especially if your base is near a fuling village. Fulings also tend to wander farther from their spawn point at night. I was having this problem with my plains base and solved it by making the perimeter of the base larger.

I am not even sure that workbenches do it now too...
I have tiny strip of Plains surrounded by black forest from 3 sides and ocean by another... it's really small and I made sure to place 3 workbenches that surely covers all of it... just wanted to make little barley and flax farm there... but deathsquitioes and fulings appear every few days and everynight anyway...

Pretty much any structure but walls/floors/roofs should block spawn. But they also trigger raids.

Sometimes creatures can spawn before such structures are loaded as map chunk loads... I guess if server is having a seizure something similar could happen with campfires. Or could be a straight up bug... i have not seen a wrong spawn in a looong wile... both SP/MP this patch/older patches. But we never build super big or dig too much.

It didn't change, must be some other kind of glitch. Or maybe there's a spot right on the edge of the radius.

i'm going to go with glitch i guess, i've been having all sorts of weird issues with this new server we started. I had a bunch of campfires layered around and they spawned right on to of them the minute it went to night.

Alot of things do. Campfires and workbenches are usually my go to items for it and in this case i was using the campfires. The original video i got all the items from is this one https://youtu.be/_C9fKFg6bmw where he goes through and tests them all. its just a year or so old so i was worried something changed when they didn't work for me.
