Caves in mountains, new food system, more building, and a couple more enemies pretty much. I'd suggest waiting for mistlands, whenever that drops.

caves really, that sounds cool. o yes i remember mistlands were there but not accessible yet

Start over with a new character. Quite a lot has changed in progression that you wouldn't see if you did an old save.

For example you need turnips to upgrade the cook pot to make sausages.
Serpent stew is now locked behind iron and chain.
Comfort upgrades changed.. some items added but others were made to be replaced by the next better... for example you don't need a brazier and stone hearth just one or the other.

Mining was made a lot faster by allowing the pick to pass through so you hit everything in the swing radius. Clearing an iron crypt goes a lot faster now.

Disposal device added

Mini bosses added

New armors added, new weapons

New areas in mountains, ice caves.

Probably forgetting a lot as well.

I'd wait another year or 5, still no biome update since release. Tiny development team that made millions and just refuse to hire any help. Definitely fun but expect progress and development is as slow as Star Citizen.

Oh they added a log seat you can build without a table. Adds to the comfort inside a crypt.

Nothing substantial has been added in a year. They wasted a lot of time putting the console version out. No loyalty has been shown to the PC players that made this game so successful at launch.

oh wow i didnt even know there was a console version, things are making sense now, free updates won't get them more money but console sales would

From my understanding they hired someone else to do the port for them.

But they haven't put a console version out...

The larger updates since launch...
Hearth and Home (this is where I started) adds a great deal of base-building options, a new mob and resource in the plains, and revamped the food system.
A smaller update in late autumn added a new mob to the swamp, a new resource, and new armor.
A medium update in late winter added mountain caves, a dungeon in the mountain. This came with new mobs, new materials, new armor (that's great for working at the base) and a few other things.
Lots of spoilers for the Mistlands update have been slowly leaked... but the delivery date on this has constantly been pushed back by the devs' ambition. They're promising a lot for this, which means they need to take the time to actually deliver, which raises our expectations, which means they need to take the time to actually deliver, and as far as I can tell we're in an infinite spiral here.

Oh. And Lox saddles. Don't forget lox saddles.
