i came back after a long break and traveled to the current boss location (The elder). I travel really far and when I get to the marker there is nothing there. No altar, no boss, nothing. Just some ruins and trees like any other part of the black forest.

Did the game misspawn the altar? Is it underground? Am i missing something?
