Answer to "does this count as loaded"
If the game has EVER revealed the location, whether it is visual render distance or map view distance, it is loaded and unchangeable. So if you've sailed a circle around a medium island, chances are the interior has loaded as well. Hope this helps!
Source: experience with Minecraft's procedural generation engine and coding. Same model and same reasons for issue.
A quick explanation. If an update brings in something that changes the terrain or other PERMANENT loaded features, the terrain existing can't be both old and new, and it cannot simply replace the old. So once you load an area the terrain is created, and that's that. If an update brings in new mobs, items etc that spawn in a certain location these changes can be seen in old areas since they are not changing the geography itself.
Hopefully this helps you decide if you want a new save or not!

No man's sky uses a VERY unique algorithm. Every time you load you are actually reloading those terrains as fresh (look at the frustration of players having terrain regenerate into base builds)
I'm over 1000 hrs in NMS and this is the thing that makes me the CRAZIEST. I have a super cool underground base which I have to clear out every time I go there. After doing it a few times I basically just abandoned it. Same with the base I built embedded in a cool pillar on another planet.
Personally, between the two approaches I actually do like VH's better. If you use this mod, and think about your builds before you make them, you can reduce the risk of losing the cool things you create quite a lot - https://www.nexusmods.com/valheim/mods/1125

Same process affects Ore Veins, if you dig only part of it and consistently leave some covered you may dig an "infinite vein"

Given that the terrain is generated from the algorithm first then player edits are applied to it, if they changed how world generation works, you wont have the old stuff as i don't believe Valheim saves the actual world state at all, just your changes.
Not that i think they will do this, they have placeholders already there for all the future planned biomes so there's no point in upsetting things.
Nearly all other games load terrain changes before base changes, and my experience tinkering on valheim makes me think it is the latter.
I'd love to be wrong on this one but it is unlikely, since the guide says new saves may be needed for a couple features.

I'm no authority on this at all, but on my 400 hrs savefile, the save time is about 15-17 seconds long. I built a decent bit around the world, but i noticed the time still increasing even when i was done building and just exploring the world in search for a Berserker trophy, and I don't think it's just the loot that makes the save file so big, so my honest assumption would be the discovered world to be saved along. Plus, a fresher world and character i'm currently playing (this time with very minimal building and terrain modification) are showing the same increase, really makes me think that's the main reason

I appreciate it. Although, from what has been said before, it seemed that a fully explored map would only lock it from new content, meaning you'd be able to play in the old world but Mistlands, etc would still be empty and not updated with the new content.I don't see much point in starting over until the game leaves the Early Access state. Maybe start a new seed then, or possibly I'll finish the game in this original seed and be done with it.
Players who have fully explored the map prior to new content will need to restart. I've seen a lot of "but what about if..." So I wanted to try to clear it up in detailed, but laymen's terms.

Correct. However new mobs may spawn there even on explored saves, while the terrain would remain "old"
