You can get true first person two ways, but it is quite limited. On a ship you can zoom down, then pan your mouse to a low angle view that is equivalent to first person, or you can sit on the ground and your view will move to first person, but you must remain seated, so no combat.

You can zoom in and out quite a bit with the mouse wheel, but no first person.

Mouse wheel moves camera in and out. You can get it fairly close.

the first person mod is actually annoying to me, I tried it. You really do kinda need that zoomed out view quite a bit especially if you're being swarmed or surrounded by mobs.

I find sailing with it all the way back better as well. Makes it easier to spot rocks and serpents as well as see around the sail.
