My first house is typically on top of a rock with part of the rock stick through the floor. Makes it nice so you can cook indoors as well.

Can build one in any lil' starter house.. just leave one floor tile open, and build it on the ground there (can even stick the floor tile under it after you get it going ;) ) - Like said elsewhere, make sure the smoke has somewhere to go though

You can build a fire in a totally enclosed room (such as under the Elder altar stone) by creating a smoke chamber over the fire. Needs to run a few tiles long but can be sealed and will contain the smoke.

Yes, but the standard fire needs to be built on ground (not on wood tiles), later game stone fire place can be placed on floors. But also keep in mind you need a kind of chimney to let the smoke get out.

Nope. But they will continue to burn in the rain and provide the fire comfort bonus if you are sheltered near one.
