For first point, you can reposition your character and chose where to fight, as a workaround.

Fighting on slopes does suck with many weapons, but some weapons work very well when hit against the ground. That is the workaround to try.
Monsters seem to hit in a hemisphere, however each weapon has a different profile, cones etc.
Very steep ground though, or ledges can be annoying.
