I've killed all 5 bosses with no deaths on a play through so you can get all the way. You do need to be cautious, always have Potions and Bonemass at the ready and before that Eikthyr so you can just escape. Also try and get enemies to fight each other especially growths on the plains as it is much safer to set them off against fulings or lox than fight a bunch of them direct. Also much easier solo as enemies have less health so you can for example one shot normal Fulings from a distance, which makes it possible to take out villages without fighting the entire mob in one go.
Personally I like the challenge, sense of achievement and thrill of the close calls when there is a real consequence to your actions. That said I have had to restart quite a few games, mainly to starred skeletons when raiding my 1st tomb.
I do play the game with mods although the only difficulty effecting mods are glutton (which just auto eats so you don't need to remember to) and one that lets you take metal through portals. So not sure it counts from a purists point of view, but it works for me. After that I have tried to repeat the challenge a few more times and currently just have the last boss to find. Although I am also slightly more forgiving on myself now as I did choose to ignore a death by falling while messing around repairing a non essential base out of combat. Something other than gravity kills me and it's still game over though and start again.

A normal perma death run is a challenge in and of itself. And I would guess few have actually done it (no mods, no cheats, just normal game mechanics).

Personally I have found the most challenging play style is a perma death, no boss run.

BUT you will get stalled in the Plains (if you can get that far) playing this way as you need the tears from the drake boss to upgrade any farther. So the best you can get is silver gear.
Still it is enjoyable to see how far you can get this way. Troll mining for the win!

Other challenges that were fun (always perma death) were trying to make it through the game using no bow (spear for ranged and yes, drake boss will land and can be melee'd) or only using one particular type of weapon all the way through (i.e. only slash, only pierce, only blunt, etc.)

In general, at least for me, perma death runs tend to be long games in 'game days' as I play a bit more cautiously than I would otherwise.

But lots of self imposed limitations you can think up.

You already CAN Starve to death. If you don't eat food for a long period of time that 25 starts ticking down to 1, and then a stiff breeze will kill you.

Constant rain would make it impossible to do most things in any biome that isn't the Swamp.

But Permadeath and always on 2 stars could be a lot of fun. No teleporters is something you can just do on your own already, just don't make them.

I guess if you want constant rain you could just type in the console command that enables it and just leave it alone, it will keep raining until you log out, making you wet the entire time.

I'm sure there are probably mods to address these conditions, but IMO, the devs shouldn't turn around and re-code the game (at least not at this stage) to do things other than what they are already working on.

I agree the game is a lot easier once a player is experienced with all the various nuances, but not a good reason to alter what they already have set in-place, at least until the whole game is fleshed out.

If you want permadeath, just delete the world after you die

Don't need god mode to beat this game. After one learns how each enemy acts / fights, it's very easy to play through all the content with very few deaths, if any at all.

Another "fun" challenge is no maps! Was pretty hilarious when we did it. Lost for days, going in the same direction. Turned maps back on just to find out you were lost in the backyard of your camp lol.
