It's rare, but sometimes, especially in dungeons, the gravestone can clip and drop into the void. This game is so polished already, people forget it is early access.

Me is host in server and i don't lost nothing. Only my friend lost gvavestone. He play with me over 250h.

Gravestones can clip into things. If you are not sure if he died on ground destory anything in the vicinity like stones bolder etc.

If you can't find it:

add -console to the steam start options
F5
enter:
devcommands
spawn XYZ

You can spawn whatever you like,, and rebuilt his inventory. Search for the list of itemnames.

Dungeons exist around 8000 meters above the map. If the gravestone clips through one, then you'll most likely find it outside at the approximate location of your death.
