I haven't played since April last year. I'm not sure how much has changed, but should I create a new character or just a new world?

Most of what you saw is probably mods and not the original game. If you're talking about tar wood items, well... get tar. :D
And be prepared to die. :D

I guess in the end its really depends on what you enjoy the most about the game.
I personally enjoy a balance of activities, so restarting is not a bad thing. I get to experiment with building new starter homes on a fresh world, do some chill hunting or resource collecting, then add in some exploring... all without really dangerous events, until I am ready.
If you prefer the combat and progression then there is not that much added, some new armours, new mobs, new dungeons... all cool but wont validate a start from scratch.
In that case it may be more worthwhile to wait for Mistlands, then restart so you have a nice chunk of game to work through with new stuff to look forward to.

I would suggest a full restart, H&H added some new stuff with food and how shields work, and it would get you nice and ready for Mistlands later when it drops.

It's nice to have an alternate character and world. You can rescue yourself and
have more resources available. There's no reason to delete your old character
or world. If you didn't explore the entire map new stuff like the Frost Caves will
spawn in unexplored areas.
It doesn't take that long to develop a new character either because you know
what you're doing and can skip some weapons/armor.

The only reason to create a new character or world is to ensure greater content of the new Frost Caves content. The addition of Frost Caves to the mountain biomes of Valheim will only be added to unexplored regions of mountain biomes, previous explored mountain regions will not have any Frost Caves added to them. If when taking a break from the game, back in April 2021, you had not explored that much of your Valheim seed map, there really is no essential reason to create a new world. There is no essential reason to create a new character.

Tbh just wait until Mistlands, the recent updates are cool, but after I played them I wish I had just waited until Mistlands so Valheim would have a much more different feeling and experience than my first time playing.

Nobody knows. I would say.. end of the year or early 2023. If not I really start to think that this is just a holiday project after getting the profits.
And yeah I also would wait until mistlands.. the new caves really arent worth a new playthrough even when some hardcore fans would say otherwise.

i would start a new world. By the time you beat the plains the mistlands might be out for you to play with. Just be careful not to go too far out.

I would probably start a new character with a new world just because progression with an old character in a new world or a new character in an old world is probably more disorienting. Like you would have recipes but not the stations to use them, or you would have the stations but not the recipes.
Most of the new content since early access began is loaded to the mid and current late game. Since you already have an old character and an old world you are in a flexible spot. You could also tailor your own compromise by using your old character to drop or build some basic stuff in a new world so that your new character can move the early game along faster, but practice some restraint.
My personal opinion is that the swamps is where the game really begins to shine and it is from that point onward that most of the new content begins to appear.
You can still find the new content in an old world but it depends on how much of your world has yet to ever be generated. New additions to the world-gen will not appear in previously generated areas.
