Well it really depends of if this is something the devs would want to spend time making. If you tunnel under a big rock in the right position, you can make a cave. So if that's possible, then they just need to make a system that allows for a second bottom surface and top of the cave. Add an option on the pickaxe, dig tunnel. But the coding behind what is simple to write isn't always simple.
