Don't think I saw it mentioned, but if you see enemies moving towards you, aim at them so you can see their name, and you'll be able to see if they can just 'hear' you (sound symbols on either side), or have actually spotted you (exclamation mark). If they just hear you but haven't spotted you yet, you can still get sneak attacks on them if you have a ranged attack.
