You need to set a tag for each portal. The two you are connecting must match exactly. I usually name my first A then next set B and so on. That way you can leave a marker on the map as A1 and A2 and keep your map sorta organized. Just my recommendation with naming since both sides must be the same and I find it keeps it easier to navigate once you go over ten portals.
