My personal preference is to always keep 1 un-connected at home, called "home" or "base", and never re-name it.
I'll go out exploring, drop a portal and name it "home", jump back to the base and create and name a new portal "6" or "7" or whatever comes next, use the home-home portal jump to the wilderness, then rename that one "6" or w/e. For me it cuts down on the occasional naming screw-ups, and means I've always got a route home.
