Not sure what you are referring to as Rancid Remains, & yeah those berserkers till the moment didn't drop me a single head though I could something +30, I assume you mean wraiths by ghosts & yeah while you don't see them often they can respawn in areas where you killed them but not sure about the rate or the condition.
