I am using a mod named "Trial of Odin" to increase my game difficulty level (both for my SP and my MP game (MP game is me and a friend). In SP I count as 2 players while in MP we count as 3 players.
I can only give an "educated guess" here but i think that the game saves any changes to individual creatures - i once removed the Trial of Odin mod due to problems and the "old buffed enemies" from the mod where still in the game and hitting much harder than the normal enemies that were spawned after removing the mod (the buffed enemies also dropped lots of stuff after i killled them but i think that has something to do with the mod itself and not with the game (unless it is "not intended" to further change enemy difficulty without having any side effects by the game itself).
I think the "server owner" is the only person who needs to make such changes (via the command console if you dont use any mods that alter the difficulty level). For dedicated servers, i think you need to be an Admin on that server (you need to enter some ID (Steam???) into a config file somewhere in the game directory (or in the character/world save directory; not 100% sure where the file is) in order to be able to execute console commands (with effects; you can always type them in....). This "guess" is based on the fact that a console LOD detail setting does not seem to work for me on my dedicated server - but i found the information about enabling my "Admin rights" by entering that mysterious ID into that mysterious file - which i never tried; i was just testing some stuff and the LOD setting is not important to me....)
(Using console commands on a local MP server gives varying results: the effects of (e.g.,) spawning trees and bushes and removing dropped items can be "seen" by everyone in the game immdiately, but changing the time of day only works for the client (i,e,, the server owner client) and not for any other clients connected to that server. As i wrote above, changing the number of players ("X") on a local MP server by the person who set up that local MP server should work for everyone connecting to that local MP server).
Please remember that these are only "educated guesses".
