I found the problem. I use Vortex. It adds files to Bepinex plugin directory. I removed all vortex files. Reinstalled Bepinex manually. Then added Enhanced CFG MGR and Equip and Quick Slots and they worked.

I conclude Vortex was hosing Bepinex.
