It can happen under sail with crew too, ships just love taking random 10hp knocks from what appears to be nothing. Definitely something to do with multiplayer lag.

Did you run into a patch of mountain biome that didn't fully develop under the sea? This happens sometimes with all biomes.

I wonder if the serverside simulations mod improves this?

Oh, I'd have to run some tests. Pretty low though, most of the actual simulation happens client side and the server just passes messages about actions by other nearby players.
