Much the same way that skeletons do not respawn in the burial chambers once cleared. I'm too afraid to go in the troll cave and haven't killed one just yet.

My understanding is that the trolls on the surface respawn, but the trolls in the troll caves do not respawn once killed.

You will only find Moder on the largest of mountain systems, not small ones by any means. Double check with https://valheim-map.world/ to see where it is.
