It's hard to make out what exactly happened from that description, but it sounds like there were more than 2 portals with "unnamed"?
Anyhow, the way I have been able to keep track of all my portals, is to build a portal "hub" near the starting circle (since it's one of the safest areas and because doing game-progression has the player returning to the starting area numerous times to place Forsaken trophies.
ALL the portals I have placed in the world, have a matching-named portal at the Hub. as well as numerous "blank", not-yet-used portals, which I simply name 1,2,3,4, etc so that they aren't activated, and most importantly, I have a permanent "unnamed" (default) portal which remains full time at the Hub.
When I go out exploring, I carry mats for a portal and if I find a place I want to set a portal I can just plop the default portal down and hop through... it takes me to my permanent unnamed portal at the Hub.
Now it's a simple matter to change one of the extra (numbered) ones and make a name for the new portal i set (in the world), then hop back through the unnamed, default portal, which takes me back to that newly-placed portal and i rename it to whatever new code i set back at the Hub.... Wait for the link to change from default to newly- named and presto I have my new portal link as well as ready to repeat that process the next time I want to place a new portal.
I never have a situation where I lose track of portals or have more than 2 named the same or get ported to the wrong places... I USED to have those issues sometimes, but I started using a dedicated portal hub, on each playthrough, at the starting location and have never looked back.
This is one such portal Hub I have on one of my many worlds... It's later in the game, after I was able to upgrade to plains resources, but it shows the idea and layout.
