I've experienced this as well I get to the tomb marker and there's nothing there agree with what FluffyBob said about map location. Look around more.
It should remain there unless you die again it will move to new death.
i'd quit the game first then verify game files then load back in and see if its appeared.
There is the devcommands to rebuild all your weapons. Look in the guides for it that at least can rescue you .
