Thats normal Valheim behaviour since the game was launched. Even in single player. The portal updates every few seconds. You can get unlucky with the timing.

In 1000 hours extensively using the unnamed pocket portal technique it literally never happened to me.

Like youdeconstruct a portal, rebuild it farther away and get teleported to where it used to be. Never seen that.

The only "bug" I've seen with portals is when you rename it and go through immediately. It sends you to the other portal with the old label.
