Usually happens to me while crafting stuff at base, as I switch rapidly between chests or something, not sure exactly.

I bet it has something to do with rapidly switching between weapons, eating and using the pickaxe. I have only seen it this one time in a few hundred hours of gameplay. Does it happen to you often?
