The dungeon mechanics can be confusing, it looks like you're leaving the world to enter an instance so you might assume time will stop outside, but the actual mechanism is you're teleporting straight up into some floating rooms. Things around the entrance stay in the simulation.

Absolutely not if it gets the drop on them, they're a one-shot kill to troll ground pound.
