This is true, but many of us are completionists and can't stop at level 1 basic weapons, we feel a need to make the highest upgrades everytime.

Are you playing solo or in a group? if multiplayer it's a known bug caused by server desync.

Yeh 6 is about max, unless you're all really close or on same network. Get's abit laggy after 6. But I have been on servers with 10 and it is fairly workable, but it is also v laggy.

You can farm Wraiths hoping for that one chain drop or just check chests in crypts where it's not uncommon to find 5 chains in a chest.
